This role involves conducting comprehensive patient assessments, developing and implementing individualized care plans, and documenting OASIS data accurately to support quality patient outcomes and reimbursement requirements.The RN collaborates closely with patients, families, and the interdisciplinary care team—including therapists, social workers, and physicians—to coordinate care, monitor progress, and provide education and support. This position requires strong clinical expertise, attention to detail, and the ability to work independently in a home care environment.QualificationsMinimal of two years of previous Home Health experienceA current license as a Registered Nurse in ConnecticutHave reliable transportation and be willing to commute to provide the best in-home patient careWhat You’ll DoPerform assigned duties, including administration of medication, wound care, treatments, and proceduresMonitor clients' conditions; reporting changes to Nurse ManagerFollow up with, execute, and properly document doctors' ordersPerform client assessments as necessaryCollaborate with other professionalsManage home health aides or CNAsWeekend Rotation will be required. Typically, this is one weekend per month or less.CompensationPay: $65.00 - $75.00 per hour
