Continental Care and Rehab is looking for a compassionate and motivated individual to join our social service department. Applicants with or without experience are encouraged to apply as training will be provided upon hire and on an ongoing basis.

The primary objective of the social service department is to advocate for the individuals receiving care in the facility while helping to promote the best possible health and well-being outcomes by operating in accordance with current federal and state guidelines and regulations. Duties include, but are not limited to, facilitating meetings to discuss the individual’s progress in all aspects of their care, completing evaluations/interviews and working with outside agencies to ensure the individual needs are being met in the facility and the community.
