This role is responsible for timely and effective clinical care, collaboration with a multidisciplinary team, spiritual outreach, and participation in organizational initiatives that promote wellness, efficiency, and patient empowerment.Key ResponsibilitiesClinical Excellence & Team-Based CareArrive and start sessions on time; prepared for AM/PM patient sessionsParticipate in and lead daily team huddlesDelegate effectively to support staff and maintain patient flowFollow up on hospital discharges, test results, referrals, and medication refillsProvide efficient, high-quality patient care, completing documentation within 24 hoursEngage in cost-effective, evidence-based care aligned with EHC clinical guidelinesPrescribe responsibly, especially regarding controlled medicationsHandle patient needs efficiently during on-call periods and complete follow-up documentationPatient Engagement & EmpowermentEducate and empower patients to take an active role in their own wellnessSchedule follow-ups strategically for chronic condition managementSet clear visit agendas and boundaries with patientsProvide culturally sensitive and spiritually integrated careEncourage patient involvement in decision-making and follow-up complianceDocumentation & Follow-UpComplete charting, referrals, and documentation on the day of the visit whenever possibleReview labs and communications bins regularlyAddress refill and message requests within 2 working daysComplete patient forms within 5 working daysSpiritual & Holistic CareProvide spiritual care through prayer, scripture, and connection to churches when appropriateRefer patients to behavioral health or social services as neededServe all patients with humility, love, and respect—regardless of spiritual interest or beliefsParticipate in daily staff devotions and contribute to the spiritual life of the teamCollaboration & CultureResolve conflicts directly and respectfully with team membersEncourage team morale and model a positive, constructive attitudeTrain, mentor, and precept students when appropriateSuggest and support clinic-wide quality improvement effortsParticipate in fundraising and community outreach events (optional)Clinical Skills & AccountabilityDemonstrate sound clinical judgment and stay current with care guidelinesMaintain up-to-date credentials (license, DEA, CME)Seek supervision appropriately and adhere to licensing limitationsFollow organizational protocols, including on spiritual care boundaries and biblical ethicsQualificationsLicensed Nurse Practitioner (NP); Family Medicine preferredAt least 2 years experience in primary careDemonstrated commitment to Christian values and ethical practicePassion for serving underserved populationsStrong communication, teamwork, and organizational skillsAbility to integrate faith into clinical care in a respectful, patient-centered mannerAccountable to and active in a Bible-teaching church congregationCOVID-19 Vaccine RequirementEsperanza Health Center requires all on-site workforce members, including employees, contractors, and volunteers, to be vaccinated against COVID-19.
