The System of Care Coordinator engages and works with children, youth, and families with significant behavioral health needs, providing care coordination activities. Integrated and coordinated care provided must be aligned with The Buckeye Ranch values, including family-centered, strengths based, trauma-informed, and culturally responsive. Team members must have a thorough understanding of local communities, be skilled at developing working relationships with community agencies, and be able to identify potential community support to assist families/caregivers working collaboratively with Child and Family Teams.
