Registered Nurse Case Manager

Job Summary:

The Registered Nurse - Case Manager is responsible for providing skilled nursing care, case management, and coordination of patient care in an acute hospital setting. This role requires expertise in acute care nursing, proficiency with Epic electronic health records, and the ability to collaborate effectively with interdisciplinary teams. The nurse will manage patient assessments, treatment planning, and ensure quality care delivery while adhering to facility protocols. This position requires at least one year of experience, an active Alaska RN license, and current BLS certification. Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed and monitor patient responses.
• Coordinate case management activities, including discharge planning and resource allocation.
• Collaborate with interdisciplinary teams to ensure continuity and quality of care.
• Document patient care accurately using Epic electronic health record system.
• Provide patient education and support to patients and families throughout the care continuum.
• Adhere to hospital policies, procedures, and regulatory requirements.
• Participate in quality improvement initiatives and staff development activities.
• Float within nursing scope to meet facility needs as required.

Required Skills & Certifications:

• Active Alaska Registered Nurse (RN) license in hand at time of offer.
• Basic Life Support (BLS) certification.
• Minimum of 1 year acute care nursing experience.
• Proficiency with Epic electronic health record system.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Ability to work effectively in a fast-paced acute care environment.
• Excellent communication and interdisciplinary collaboration skills.

Preferred Skills & Certifications:

• Acute Care Management (ACM) certification preferred.

Special Considerations:

• Weekend work required: every other weekend or 4 weekend days within a 4-week schedule.
• No on-call responsibilities.
• Only one time-off request allowed per 13-week assignment, limited to 5 days or less; no holiday time off for auto offer candidates.
• Floating within scope of practice to meet facility needs is expected.
• Local candidates must reside at least 50 miles from facility; pending licenses are not accepted for auto offer.

Scheduling:

• Shift options include 4x10 or 5x8 day shifts.
• Shift times vary and may be one of the following: 0700-1530, 0730-1600, 0800-1630, 0830-1700, 0700-1730, or 0730-1800.
• Guaranteed minimum of 72 hours every two weeks; one 8-hour shift may be called off every two weeks.
• Block scheduling accommodations are not available.
