Requirements:

• Proficient in METs testing evaluating and diagnosing respiratory and cardiovascular
• Own a laptop (no Mac/Apple products)
• AANP
• Active and unrestricted full license
• Willing to complete DMA training to become DMA certified
• CV needs current home address listed for presentation
• Willing to perform Gen/Med
• DMA certified
• Open to travel to other locations
• Does provider have their own office? If so, provide address
• Self-Query NPDB (Should be pulled within the month)
• Is the provider active military duty in the reserves or actively employed at the VA? (yes or no)
• Number of days per week available for VES?
• Are there specific days of the week provider is available for VES?
• Copy of ECFMG certificate if attended school in a different country
• Self-Query NPDB (should be pulled within 30 days)
