The Enlightened Mind Counseling Center is seeking a dedicated Licensed Master Social Worker to join our compassionate and dynamic team. Work Environment

This role is structured as a remote position that allows for flexibility while working within a community health setting. Operate primarily during standard office hours from Monday to Friday, between 8:00 AM and 4:30 PM, while maintaining the readiness to accommodate evening or weekend hours based on urgent client needs. As part of our commitment to continuous care, on-call availability may also be required occasionally.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments of seniors that evaluate their emotional, social, and mental needs, allowing for tailored support.
  • Develop and implement individualized care plans collaboratively with clients and their families, targeting specific objectives and preferences.
  • Provide empathetic counseling and psychological support to seniors, assisting them in navigating life transitions, challenges, and adjustments.
  • Act as an advocate for clients, ensuring their rights are upheld and that they have access to necessary community resources and services.
  • Engage in interdisciplinary collaboration with healthcare professionals—including nurses and caregivers—to ensure comprehensive care coordination.
  • Regularly visit clients to monitor their well-being, assess living conditions, and address safety concerns.
  • Facilitate effective communication and conflict resolution among family members involved in the care of senior clients.
  • Educate clients and their families about community resources and support groups that can bolster their social engagement and enhance their overall quality of life.
  • Maintain detailed documentation of client assessments, care plans, progress notes, and other relevant interactions to ensure continuity of care.
  • Stay current with social work best practices and ensure compliance with local regulations and ethical standards.

Patient Population / Client Focus

The focus of this position is on supporting elderly clients, some of whom may require assistance with complex social issues related to aging, mental health challenges, and life transitions. This role also encompasses working with families to navigate the emotional landscape of caring for aging loved ones, reinforcing the importance of a supportive network.

Qualifications

  • A Master's Degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Master Social Worker (LMSW).
  • A minimum of two years of relevant experience in social work roles, especially within community health settings or working with geriatric populations.
  • Strong knowledge of social work principles, issues pertinent to aging, and the available community resources that can assist seniors.
  • Exemplary assessment, counseling, and problem-solving skills, with the ability to empathize and connect with clients and their families.
  • Exceptional communication and interpersonal skills, ensuring effective interaction with interdisciplinary teams.
  • Familiarity with electronic health record (EHR) systems and basic computer applications.

Preferred Experience

  • Previous experience in a community health clinic or similar setting that focuses on senior care can be highly beneficial.
  • Exposure to interdisciplinary teamwork in healthcare environments will be advantageous.
  • Background in providing educational workshops or community program facilitation is a plus.

Compensation and Benefits

The salary range for this position is competitive, ranging from $60,000 to $100,000 annually, depending on experience and qualifications. Additionally, comprehensive benefits include:
