Working collaboratively with the hospice care team, the Licensed Practical Nurse (LPN) serves as a valuable resource to patients, families, colleagues, and community partners by providing information, guidance, and support.This position is responsible for assisting in the delivery of high-quality, patient-centered hospice care while ensuring compliance with agency policies, regulatory requirements, and established clinical standards. The Licensed Practical Nurse (LPN) contributes to the achievement of organizational goals by promoting efficiency, continuity of care, and exceptional service. The primary focus of this role is to facilitate the highest quality patient care through comprehensive, compassionate, and consistent support for patients and their families throughout their hospice journey.The Licensed Practical Nurse (LPN) at Beloit Regional Hospice will:Exhibit each of the Beloit Health System Behavior Standards and role modeling excellence for all to seeProvide telephone calls and patient triage supportSupply and DME (durable medical equipment) orderingComplete audits, documentation review, and related follow-upComplete Patient visits as neededAssist with and participate in marketing opportunitiesSupport RN case managers with patient needs and follow up to phone callsAssist clinical manager with staff training, competencies, meeting minutesProvide coverage for palliative care for vacation and ill-time coverageJob Requirements:Graduate of accredited practical nursing programActive Wisconsin LPN at time of hireIllinois LPN preferred, if not active IL at hire, must apply for IL License within 30 days of hire2 years' experience in Palliative Care or Hospice preferredDemonstrated ability to interact with wide variety of medical, professional and community groupsComputer proficiency and strong public speaking skillsMust have valid driver's licenseMust have and maintain auto insurance coverage that meets requirements and maintains reliable vehicleAbility to work independently and make autonomous decisionsCurrent CPR requiredApply today!
