Hospice Transitional Nurse Liaison

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
• Obtain complete and accurate demographic information and medical history, including diagnosis and primary care physician information
• Introduce the agency to patients/caregivers, explain services, and coordinate start-of-care visits
• Order requested DME, supplies, and medications related to hospice care
• Deliver community education for assisted living facilities, skilled nursing facilities, and hospitals as requested
• Provide bedside patient education regarding hospice, disease progression, and end-of-life care
• Attend discharge planning meetings and work with facility teams to support safe, successful discharge home
• Determine hospice eligibility and appropriate level of care
• Communicate with the clinical team regarding complex, chronically ill patients to help prevent re-hospitalizations
• Partner with field staff and leaders to support patients

Requirements Associate degree from an accredited School of Nursing Minimum of two (2) years of experience working as a Registered Nurse, including acute care or oncology experience Hospice experience preferred Experience with Microsoft Office Suite, EPIC, and HCHB preferred Able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety Ability to accept different lifestyles, cultures, beliefs, and values Proficiency with computers Successful completion of a drug screen prior to employment Legally authorized to work in the United States Core Competencies Demonstrates expertise in patient assessments, hospice care coordination, and effective communication with clinical teams. Proficient in utilizing healthcare technologies and providing education to patients and families regarding end-of-life care.
