Family Medicine Nurse Practitioner

Position Overview

The Family Medicine Nurse Practitioner will provide comprehensive outpatient primary care services across the lifespan. The provider will focus on preventive care, chronic disease management, acute care visits, and patient education while working collaboratively with physicians, nurses, and support staff.

Responsibilities

• Conduct comprehensive patient assessments and physical examinations
• Diagnose and treat acute and chronic medical conditions
• Develop and implement patient-centered treatment plans
• Prescribe medications and manage ongoing therapies
• Provide preventive care and wellness counseling
• Order and interpret diagnostic tests and lab work
• Collaborate with physicians and interdisciplinary care teams
• Maintain accurate documentation within EMR systems
• Participate in quality improvement and patient satisfaction initiatives

Qualifications

• Certified Nurse Practitioner with Family Medicine focus
• Active or eligible Connecticut NP license
• Strong communication and patient care skills
• Ability to work effectively in a collaborative outpatient setting
• New graduates and experienced Nurse Practitioners are encouraged to apply
