Home Health Physical Therapist

CenterWell Home Health is seeking a compassionate Home Health Physical Therapist to join our Healthcare & Medical Services team, delivering skilled, one-on-one rehabilitative care in the comfort of patients’ homes. In this role, you will evaluate adult and geriatric patients, develop and implement individualized plans of care, and provide evidence-based therapeutic interventions designed to improve mobility, strength, pain management, and overall functional independence. You will collaborate closely with nurses, occupational and speech therapists, physicians, and caregivers to ensure safe, coordinated, and comprehensive care that aligns with CenterWell’s focus on patient autonomy and quality of life. Daily responsibilities include performing thorough physical therapy assessments, setting measurable functional goals, educating patients and families on home exercise programs and safety, monitoring progress, and adjusting treatment plans as needed. You will complete accurate and timely documentation using electronic medical records, communicate changes in condition to the interdisciplinary team, and participate in care conferences to support optimal outcomes. As a trusted clinical resource in the home environment, you will promote fall prevention, safe transfers, proper use of assistive devices, and strategies that allow patients to remain safely at home.
