Home Health Physical Therapist Summary: MLR is seeking a dedicated and skilled Home Health Physical Therapist to join our team. In this role, you will provide in-home therapy services to help patients improve mobility, strength, and overall physical function. You'll work closely with patients and their families to develop personalized treatment plans aimed at enhancing independence and quality of life. Home Health Physical Therapist Key Responsibilities:

• Patient Evaluation: Assess patients' physical capabilities and functional limitations in their home environment, and create individualized treatment plans to improve mobility, strength, and overall function.
• Treatment Planning & Execution: Implement therapy programs that focus on improving movement, balance, and independence with daily activities.
• Patient Education: Educate patients and caregivers on exercises, home modifications, and strategies to promote mobility and safety.
• Collaboration: Work closely with physicians, occupational therapists, nurses, and other healthcare professionals to ensure coordinated, holistic patient care.
• Documentation: Maintain accurate, up-to-date records of patient progress, therapy sessions, and outcomes in compliance with home health regulations.

Home Health Physical Therapist Qualifications:

• Education: Graduate of an accredited Physical Therapy program.
• Certification: Current state licensure as a Physical Therapist (PT).
• Experience: Previous experience in home health or outpatient therapy preferred, but new graduates with strong clinical skills are encouraged to apply.
• Skills: Excellent interpersonal and communication skills, strong time management, and the ability to work independently while delivering high-quality care in patients' homes.
