Responsibilities:

Administer medications, treatments, and therapies as prescribed by physicians

Monitor patients' conditions, reporting any changes or concerns to nursing staff

Provide personal care to patients, including bathing, dressing, and grooming

Collaborate with interdisciplinary teams to develop and implement individualized care plans

Participate in patient education and training to promote independence and well-being

Maintain accurate and confidential medical records

Assist with routine cleaning and maintenance of patient areas

Requirements:

Current LPN license in the State of New Jersey

Strong communication and interpersonal skills

Ability to work effectively in a fast-paced environment

Familiarity with electronic medical records (EMRs)
