Responsibilities and Duties:

Deliver direct patient care through office visits, telehealth, and occasional same-day appointments.
Conduct comprehensive assessments, order and interpret labs|imaging, and diagnose both acute and chronic conditions.
Develop personalized treatment plans that incorporate lifestyle changes, supplements, hormone therapies, and prescriptions.
Educate and empower patients on preventive health and functional medicine principles.
Collaborate with internal health coaches, nutritionists, mental health providers, and other clinical support staff.
Ensure timely and compliant documentation of visits in the EHR (NextGen).
Participate in team meetings, quality improvement initiatives, and training sessions.
Contribute to a positive, team-oriented clinic environment.
Required Qualifications:

Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain).
Board certification in Family Medicine, Internal Medicine, or a relevant specialty.
At least two years of clinical experience is preferred; experience in functional or integrative medicine is a plus.
A strong interest in preventive, functional, and lifestyle medicine.
Excellent communication and patient relationship skills.
Comfortable working in a fast-paced, collaborative environment.
Proficiency with electronic health records; experience with NextGen is a plus.
