About the Position

Job Title: Care Manager Profession: Registered Nurse Specialty: Emergency Department Duration: 14 weeks Shift: 9:00 AM - 5:00 PM Hours per Shift: 35 Experience: - Minimum of five (5) years of clinical experience as a Registered Nurse. - Experience with care coordination, utilization review, and discharge planning. - Computer literacy is required. License: - RN licensed as a Registered Nurse in the State of New York. Certifications: - PRI and Screen certification within three months of employment. - BLS certification required. - Primary source verification of license and registration of Registered Professional Nurse from the appropriate state department. Must-Have: - Associates Degree (ASN, ADN) from an accredited Nursing Program. Description: The Care Manager will provide support and oversight in care coordination and discharge planning. The candidate will be responsible for conducting utilization review activities. Strong computer skills are necessary for documenting patient care and communicating with team members. The role requires demonstrated leadership experience. Candidates must possess an active Client RN license. Certification in PRI and Screen must be obtained within the first three months of employment. The role involves working collaboratively with other healthcare professionals to ensure quality patient care. Effective communication skills are essential for interacting with patients and families. This position will focus on improving patient outcomes and streamlining discharge processes. Leadership experience is preferred but not mandatory.
