Case Manager_

Across our 60+ locations, we provide programs in a variety of service areas, including workforce development, children, youth, families, seniors and homeless prevention. Join our dedicated team at BronxWorks Adult Protective Services program and be a part of a vital mission that makes a difference in the lives of vulnerable adults. The primary goal of the program is to provide timely and effective intervention for at‑risk adults living within the community, prevent harm, and foster an environment of safety and security through comprehensive assessment and intervention tailored to individual needs. Responsibilities Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow‑up. Manage a case load and monitor and document client progress toward service plan goals. Monitor risk and assess the status of current service plans, revising plans as client needs change. Assess and refer clients to appropriate services, including deep cleaning, home health services, legal services, mental/medical care. Utilize motivational interviewing to complete assessments and service plans. Ensure service plans are completed within 30 days and reviews occur every six months. Maintain familiarity with program resources available on‑ and off‑site. Travel all five boroughs in NYC. Coordinate client services with other social service, mental health, medical, employment, educational, child care and other providers. Conduct home visits as required and maintain weekly telephonic communication. Complete program reports monthly or more frequently as required. Seek guidance from supervisor when clarification is needed. Review and sign off on service plans and psycho‑social assessments as assigned. Report to, and meet with, supervisor monthly. Complete initial home visits within 24‑48 hours of assignment. Document all communication within 24 hours in the ETO database. Become a BronxWorks approved driver and operate an agency vehicle. Perform additional duties as assigned. Qualifications Bachelor's degree in social work or related field. 4 years of experience in case management or a related field. Certified child abuse and neglect mandated reporter, or ability to obtain certification within 3 months of employment. Enrollment in an MSW program preferred. Clean, valid driver’s license and willingness to drive a company vehicle. Engaging personality and detail‑oriented skills essential. Strong oral and written communication, time management and organizational skills. Proficiency in Microsoft Office and standard business technology. Skills / Abilities Use a computer for prolonged periods. Occasionally lift and move up to 10 pounds. Stand, walk, or sit for long periods. Climb five flights of stairs, if required, for home visits. Travel in New York City boroughs and adjacent counties via public transportation. Bend and retrieve objects and/or documents.
