Key Responsibilities Medical Record Oversight : Maintain complete and organized electronic/paper records; monitor documentation and delinquent charts; ensure timely completion. Maintain records in a state of continuous survey readiness and assist leadership with document retrieval during CMS, Alabama DMH, and other regulatory surveys. Physician/Provider Deficiency Tracking : Monitor and track incomplete physician documentation, verbal orders, signatures, psychiatric evaluations, and discharge summaries. Regulatory Compliance : Ensure adherence to CMS Conditions of Participation, HIPAA, Alabama Department of Mental Health requirements, and retention requirements; support audits and surveys. Release of Information : Process ROI requests; verify legal authorizations; maintain disclosure logs. Quality & Documentation Audits : Audit records for completeness; track deficiencies; support QAPI and corrective action plans. Behavioral Health Documentation : Manage consents, commitment paperwork, guardianship documents, court orders, and patient rights forms. Staff Support & Education : Educate staff on documentation standards; serve as a resource for record management. Reporting : Generate compliance reports; track documentation indicators for quality improvement. Qualifications Required High School Diploma or GED 2+ years in medical records, HIM, or healthcare administration Knowledge of HIPAA and confidentiality standards Proficiency with EHR systems Strong organization and attention to detail Preferred Associate’s or Bachelor’s in HIM, Healthcare Admin, or related field Experience with behavioral health, psychiatric hospitals, or acute care settings strongly preferred.
