Wound Care Nurse (RN) – Skilled Nursing & Rehabilitation Sheridan Health & Rehab | Kenosha, WI Full-Time | Registered Nurse (RN) | Wound Care Certification Preferred Sheridan Health & Rehab is seeking an experienced and compassionate Wound Care Nurse (RN) to join our clinical leadership team in Kenosha, Wisconsin. This is an excellent opportunity for a Registered Nurse with a passion for wound management, skin integrity, infection prevention, and post-acute care. Our facility is committed to providing exceptional short-term rehabilitation and long-term care services, and we are looking for a skilled clinician who can lead wound care initiatives while improving resident outcomes and quality measures. If you are a Wound Care Nurse, Treatment Nurse, Wound Care Coordinator, Skin Integrity Nurse, RN Supervisor, MDS Nurse, Clinical Nurse Specialist, or Registered Nurse with wound care experience , we encourage you to apply. Key Responsibilities Conduct comprehensive wound assessments and develop individualized treatment plans Manage and monitor pressure injuries, surgical wounds, diabetic ulcers, vascular wounds, and other complex skin conditions Perform wound measurements, documentation, and progress evaluations Collaborate with physicians, nurse practitioners, therapy staff, and interdisciplinary team members Monitor skin integrity throughout the facility and implement prevention strategies Ensure compliance with wound care protocols and regulatory requirements Educate nursing staff on wound prevention, treatment, and best practices Track wound healing outcomes and quality measures Participate in infection prevention and quality improvement initiatives Maintain accurate clinical documentation within the electronic medical record Qualifications Current Wisconsin Registered Nurse (RN) license in good standing Previous wound care experience in a skilled nursing facility, rehabilitation center, hospital, or long-term care setting preferred Wound Care Certification (WCC, WOCN, CWCN, or equivalent) preferred but not required Strong clinical assessment and documentation skills Knowledge of skin integrity management, pressure injury prevention, and infection control practices Ability to work collaboratively with interdisciplinary teams Excellent communication and leadership abilities Why Join Sheridan Health & Rehab?
