Occupational Therapist – PRN Palomar Vista Healthcare Center – Escondido, CA Palomar Vista Healthcare Center is a post-acute care facility with long-term care, short-term care, and geriatric outpatient services. We are seeking a PRN Occupational Therapist to join our in-house therapy team. As part of this team, you will have opportunities for training and continuing education in Think Thin/dysphagia, Abilities Care and dementia, stroke and neuro rehabilitation, and respiratory recovery programs. Key Responsibilities Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards. Utilize standardized assessments and evidence‑based practice to support clinical interventions. Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements. Administer various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc. Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluate treatment outcomes for effectiveness. Modify services in relation to patient evaluation and make referrals based on outcomes. Provide consultation and counseling to patients, families, caregivers, and other service providers related to the physical disorders. Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans. Provide a comprehensive discharge summary of services provided per regulatory and clinical requirements. Qualifications Licensed Occupational Therapist. Open to applicants of all experience levels.
