Registered Nurse Assessment Coordinator (MDS/RNAC) - Henrico, VA

Parham Healthcare & Rehabilitation Center is seeking a skilled and compassionate Registered Nurse Assessment Coordinator (MDS/RNAC) to join our team in Henrico, VA. As a leader in short-term and long-term skilled rehabilitation, we provide exceptional care to our patients and residents.

The Lead Registered Nurse Assessment Coordinator (RNAC) is responsible for the timely and accurate completion of the Minimum Data Set (MDS) assessments and coordination of the Resident Assessment Instrument (RAI) process in accordance with federal, state, and facility guidelines.

Responsibilities:

• Schedule and lead interdisciplinary team (IDT) care plan meetings.
• Implement and interpret MDS (Minimum Data Set) assessments.
• Review medical records, nursing documentation, and therapy notes to validate MDS accuracy.
• Accurately code MDS Items to reflect resident acuity, ensuring appropriate PDPM classification.
• Track and manage Case Mix Index (CMI) and Quality Measure data to support facility reimbursement and survey readiness.
