We are seeking a compassionate, relationship-driven Social Services Coordinator to help residents and their families navigate life's transitions with dignity, empathy, and confidence. What You'll Do As the Social Services Coordinator, you will: Build meaningful relationships with residents and their families. Assess residents' social, wellness, and support needs. Coordinate transitions throughout our continuum of care, including Independent Living, Assisted Living, Memory Care, Skilled Nursing, and Rehabilitation. Connect residents with internal and external community resources. Advocate for residents while serving as a liaison between residents, families, physicians, and community departments. Monitor resident wellness and proactively identify opportunities for additional support. Coordinate health education programs, support groups, and wellness initiatives. Participate in interdisciplinary meetings to promote resident-centered care. Help create a welcoming experience for new residents through orientation and ongoing support. Promote resident engagement and overall well-being through collaboration and compassionate service. What We're Looking For We're seeking someone who is: Compassionate, empathetic, and resident-focused An excellent communicator and relationship builder Highly organized with exceptional follow-through A collaborative team player who enjoys working across departments Skilled at problem-solving and navigating complex situations with professionalism Passionate about helping older adults live their best lives Qualifications Bachelor's degree in Social Work or a related field required. Experience in senior living, healthcare, case management, social services, or care coordination preferred. Strong interpersonal, organizational, and communication skills.
