Hospice Response Team Licensed Practical Nurse (LPN) PT Day Providence, RI, US About This Hospice Response Team Licensed Practical Nurse (LPN) Position HopeHealth is seeking a Hospice Field LPN for its affiliate HopeHealth Hospice & Palliative Care. The Response Team RN will respond to urgent patient needs and require excellent clinical assessment skills and an ability to quickly identify the best course of action to treat patients. This is not a Case Management position and requires an RN who is comfortable in an action-oriented role who is comfortable assessing and treating a patient, then moving on to the next need. This is a part time position with 24-hours per week. The hours are 8:00am-4:30pm with a rotating weekly schedule. Week 1 Sunday, Tuesday and Friday Week 2 Monday, Thursday and Saturday and every-other weekend rotation, with (1) major and (1) minor holiday. What will you do as a Hospice Response Team Licensed Practical Nurse (LPN) ? Initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living. Implements the individualized plan of care and recommending revisions to the plan as necessary and plans for discharge/transfer as appropriate. Consults with and educates the patient/family/caregiver regarding the disease process and teaches the patient/family/caregiver self-care techniques, as appropriate. Provides medication, diet and other instructions as ordered by the physician. Initiates appropriate preventive and rehabilitative nursing procedures. Collaborate in the coordination of all patient/family services and prioritization of needs with the members of the interdisciplinary team to develop and revise patient's individualized plan of care. Informs the physician and other personnel of changes in the patient's needs and outcomes of intervention, updating Plan of Care as needed. Collaborate in the coordination to determines scope and frequency of services needed based on acuity and patient/family needs. Coordinates care with other interdisciplinary team members in cooperation with the family/caregiver to meet identified needs of the patient/family/caregiver; including psychosocial, spiritual, and emotional needs and refers to appropriate disciplines/services.
