Responsibilities
- Provide direct nursing care to residents in a long-term care setting, including monitoring and documenting vital signs and health status.
- Administer medications and treatments as prescribed and under appropriate supervision, following all safety and regulatory guidelines.
- Assist residents with activities of daily living (ADLs) and support mobility, nutrition, and personal care needs.
- Collaborate with RNs and other members of the care team to implement and update individualized care plans; document changes accurately.
- Ensure adherence to infection control practices, safety protocols, and quality of care standards; respond promptly to emergencies or changes in resident conditions.
- Communicate clearly with residents, families, and team members to coordinate care and deliver compassionate, person-centered nursing.

Requirements
- Active Licensed Practical Nurse (LPN) license in New Hampshire
- Experience in long-term care or skilled nursing settings preferred
- Ability to work a 12-hour day shift and participate in a travel assignment
- Strong documentation, communication, and teamwork skills
- Eligibility to practice in a licensed long-term care environment and comply with applicable regulations
