Competitive compensation \n Pay is based on experience and education, with a range between $ 88k- $105k \n With medical, dental, and vision insurance PTO and a matching 401K \n Up to a $5,000 Sign-On Bonus to Welcome you \n A Day in the Role \n As a Home Health Registered Nurse , you will: \n \n \n Complete initial and ongoing nursing assessments in patients’ homes \n \n Manage an assigned caseload and coordinate each patient’s care \n \n Develop, implement, and update individualized Plans of Care \n \n Provide skilled nursing treatments in accordance with physician orders \n \n Monitor patients’ conditions, medications, symptoms, and responses to treatment \n \n Identify and promptly communicate changes in condition or safety concerns \n \n Educate patients and caregivers about medications, treatments, diagnoses, and self-care \n \n Coordinate services with physicians, therapists, social workers, aides, and other care partners \n \n Help patients and families access appropriate clinical and community resources \n \n Take proactive steps to reduce avoidable hospitalizations and emergency department visits \n \n Maintain accurate, timely, and complete clinical documentation \n \n Follow all clinical, safety, regulatory, and HIPAA requirements \n \n You’re a Great Fit If You Have \n \n \n A degree or diploma from an accredited nursing program \n \n A current, unrestricted Registered Nurse license in the applicable state \n \n At least one year of nursing experience in a clinical care setting \n \n Strong assessment, communication, organization, and care-coordination skills \n \n The confidence to work independently while remaining connected to a clinical team \n \n A dependable vehicle, valid driver’s license, and current automobile insurance \n \n The ability and willingness to travel throughout the assigned territory \n Physical Requirements: Sit, stand, bend, lift, and move intermittently and be able to lift 50–100 lbs. \n \n Ready for Nursing That Feels More Personal? \n Join a team that believes the best care happens where patients feel most comfortable—at home.
