Physical Therapist - Tigard, OR

FOX Rehabilitation is growing in Tigard, OR and is looking for passionate, licensed Physical Therapists to deliver exceptional one‑on‑one care to older adults wherever they live. This role takes place in Senior Living Community partners, allowing therapists to build strong, lasting relationships while managing their own treatment schedules. Responsibilities Provide one‑on‑one physical therapy to older adults in private residences, senior living communities, independent living, assisted living, and memory care. Perform evaluations, develop individualized care plans, and deliver evidence‑based treatment. Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence. Document effectively within FOX’s EMR system in accordance with regulatory standards. Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and senior living staff on safety, function, and aging in place. Provide telehealth or virtual care when appropriate to extend access and flexibility. Qualifications Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain. Degree from an accredited Physical Therapy program. Basic computer and EMR proficiency. Current CPR certification. Physical Requirements Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly. Perform CPR. Provide moderate to maximum assistance with patient transfers and bed mobility. Reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities.
