Eddy SeniorCare in Latham, NY is seeking an experienced RN Case Manager to join our community-based PACE program, coordinating care for participants to help them stay at home with comprehensive health and social services. The ideal candidate holds a NYS RN license, with at least 1 year in homecare or community health, strong communication and organizational skills, and willingness to travel within the service area.
