Requirements
- Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required.
- Healthcare Provider Level BLS certification required.
- Valid driver's license and auto liability/property damage insurance required by law.
- Oregon LCSW (Licensed Clinical Social Worker) preferred.
- Experience as a social worker in a hospital or medical setting preferred.

Roles & Responsibilities
- Provide emotional and psychological support to patients and families during the hospice process.
- Assess patients' needs and develop care plans in collaboration with the healthcare team.
- Offer counseling and social services to assist families in coping with illness and loss.
- Assist with bereavement support and guide families through the end-of-life process.
- Coordinate care and resources to ensure comprehensive support for patients and families in various living environments.
