A.O. Fox Nursing Home is a 130-bed hospital-based long-term care facility. We offer post-acute rehabilitation to get community members back home and make a welcoming home for those requiring 24-hour residential skilled nursing care. The Licensed Practical Nurse and LPN Advanced, provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician's Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. The LPN, Advanced serves as a resource for other team members and is active in the orientation of new staff. May be required to make home visits.

Responsibilities:

• Provides direct and indirect nursing care individualized to the patient's needs.
• Collects and records relevant patient health data accurately.
• Evaluates health data to identify patient problems.
• Implements and documents specified interventions.
• Identifies deviations from expected patient outcomes.
• Adheres to approved, research-based practice standards.
• Directs and supervises patient care provided by other LPNs and support personnel.
• Performs and documents Point of Care testing and quality control.
• Maintains and documents testing reagents/supplies and equipment.
• Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights.
• Delivers care in a non-judgmental and non-discriminatory manner.
• Assures that patients have informed consent signed for indicated procedures.
• Maintains current licensure, certification, and competencies.
• Provides proof of current BLS validation to supervisor.
• Completes required unit credentialing according to departmental policy.
• Practices within the New York State-defined LPN scope of practice.
• Acquires and maintains current knowledge and skills in his/her area of practice.
• Participates in required number of hours educational activity related to job function.
• Demonstrates operational problem-solving ability.
• Performs similar or related duties as requested or directed.
• Performs other duties as requested and observed by supervisor or manager.

Education:

• Graduation from a Practical Nursing program, required

Experience:

• Previous healthcare experience, preferred

License/Certification:

• Current NYS licensure as a Practical Nurse or valid interim permit, required
• BLS certification within 6 months of hire, required
• Additional credentials are determined by specific work unit, required

Physical Requirements:

• Light Work - Exerting up to 20 lbs of force frequently, and/or a negligible amount of force constantly to move objects.

• Frequent hearing, seeing/monitor/computer use, standing, talking, visual acuity (color, depth, perception), walking

• Frequent noise

Travel:

• Occasional Travel - The job may require travel from time-to-time, but not on a regular basis

Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards.
