Job Responsibilities

• Delivering quality nursing care across post-acute facilities
• Adhering to established resident care plans
• Reviewing and staying current on resident charts and medical records
• Tracking residents' physical and mental wellbeing and routinely monitoring vital signs
• Conducting intakes and carefully documenting residents' medical histories and health concerns
• Properly verifying and administering medications while monitoring their impact on residents
• Taking part in IV setup, immunization administration, and phlebotomy procedures
• Supporting the maintenance of a safe and welcoming facility environment

Qualifications and Skills

• Current and applicable state-issued LPN license
• Completion of an accredited LPN program
• A minimum of 6 months of experience in an LPN or related role
• Ability to lift a minimum of 50 lbs
