As a Licensed Practical Nurse (LPN) at Walnut Creek, you will be part of an interdisciplinary team of compassionate caregivers, working together to meet the unique healthcare goals of each resident.

About the Role:

We are seeking a dedicated and skilled LPN to join our nursing team. As an LPN at Walnut Creek, you will be responsible for providing high-quality nursing care to our residents, while also promoting a culture of compassion, respect, and dignity. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and other healthcare professionals.

Responsibilities:

• Provide direct care to residents, including administering medications, monitoring vital signs, and assisting with activities of daily living (ADLs)

• Develop and implement individualized care plans in collaboration with other members of the healthcare team

• Conduct resident assessments, identify care needs, and develop and implement interventions to address those needs

• Maintain accurate and up-to-date records of resident care and observations

• Participate in interdisciplinary team meetings to discuss resident care and address any issues or concerns

• Maintain a safe and clean environment for all residents, adhering to infection control policies and procedures
