Licensed Practical Nurse (LPN)

As an LPN, you will play a vital role in delivering high-quality patient care to our residents. Our team is dedicated to providing compassionate and personalized care, and we are seeking a strong and compassionate healthcare professional to join our team.
About the Role:
Provide direct patient care to our residents, including administering medications, wound care, and other treatments as prescribed by physicians
Collaborate with other healthcare professionals, including registered nurses, doctors, and therapists, to develop and implement comprehensive care plans
Monitor patient condition and report any changes or concerns to the nursing staff and medical team
Maintain accurate and detailed records of patient care and treatment
Responsibilities:
Provide basic care and assistance with daily living activities, such as bathing, dressing, and feeding
Monitor vital signs and perform routine checks on patients
Assist with ADLs (Activities of Daily Living) such as toileting, transferring, and mobility
Support and encourage patients to take an active role in their care and well-being

Requirements:
Current LPN licensure in the state of Pennsylvania
High school diploma or equivalent required; Associate's degree or higher preferred
Minimum of 1 year of experience as an LPN in a long-term care or acute care setting
