Job Summary:

At NuChoice Health Partners, we are dedicated to providing exceptional home care services to our valued clients in Grove City, PA and surrounding areas. As a Direct Caregiver with our team, you will play a vital role in delivering compassionate and personalized care to our clients in the comfort of their own homes. We are seeking a caring and trustworthy individual to join our team of Care Providers and provide companionship, meal preparation, laundry, bed baths, and medication reminders to our clients.

Responsibilities:

• Provide companionship to clients, engaging in activities such as conversation, games, and outings

• Assist with meal preparation, cleaning, and laundry

• Provide bed baths, incontinence care, and personal hygiene assistance

• Remind clients to take medication as scheduled

• Monitor clients' conditions and report any changes or concerns to the Care Team

• Maintain accurate and detailed records of client care and services provided

• Collaborate with other Care Providers and healthcare professionals to ensure seamless care transitions

• Demonstrate sensitivity, compassion, and respect for clients' dignity and privacy

Requirements:

• High school diploma or equivalent required

• 0 years of experience in the healthcare industry (training provided)

• Ability to lift 50 pounds or more, with or without assistance

• Must be able to work varying shifts, including evenings, weekends, and holidays

• Reliable transportation and a valid driver's license (if applicable)

• Ability to pass a background check and obtain necessary certifications
