Responsibilities
- Conduct PT evaluations and develop treatment plans for residents in a skilled nursing setting.
- Deliver therapy services, implement prescribed plans of care, monitor progress, and adjust interventions as needed.
- Document assessments, progress notes, and discharge planning in accordance with regulatory and facility requirements.
- Collaborate with the interdisciplinary team to support mobility, functional goals, pain management, and safety.
- Ensure patient safety, apply infection control practices, and maintain equipment and documentation in a compliant manner.
- Participate in care conferences and provide education to residents and families as appropriate.

Requirements
- Active Physical Therapist license in North Carolina (unrestricted)
- Doctor of Physical Therapy (DPT) degree
- Experience in a skilled nursing facility or geriatric setting preferred
- Ability to work in a long-term care environment and collaborate with a multidisciplinary team
- Strong assessment, plan-of-care development, and documentation skills
