Position Overview

We are seeking an experienced Clinical Pharmacist I to support pharmacy clinical programs by conducting comprehensive medication reviews and identifying opportunities to improve medication therapy and member outcomes.

The Clinical Pharmacist will review member medical information and pharmacy claims to identify gaps in care, drug therapy problems, medication-related risks, and opportunities for clinical intervention. The pharmacist will provide medication-related clinical support, monitor prior authorization activity, and contribute to pharmacy clinical programs serving Medicaid and Medicare populations .

This is a temporary 3-month remote assignment supporting the expansion of an existing pharmacy clinical program.

Key Responsibilities

• Conduct comprehensive Medication Therapy Management (MTM) reviews for eligible members.
• Review member medical records, medication profiles, pharmacy claims, and clinical information to identify medication-related issues.
• Identify gaps in care, drug therapy problems, medication duplications, interactions, adherence concerns, and other clinical opportunities .
• Apply clinical pharmacy knowledge and critical thinking to evaluate medication therapy and recommend appropriate interventions.
• Develop and support clinical criteria for medications and pharmacy programs.
• Monitor and review prior authorization requests as assigned.
• Provide clinical pharmacy support to internal departments and respond to medication-related questions.
• Support development and implementation of Drug Utilization Review (DUR) programs for Medicaid and Medicare populations.
• Assist with pharmacy clinical programs designed to improve medication safety, quality, and appropriate utilization.
• Participate in the coordination of Medicare MTM programs .
• Support case management teams with clinical pharmacy expertise, including participation in clinical rounds and presentations when needed.
• Help maintain and follow pharmacy department policies, procedures, and clinical guidelines.
• Identify opportunities for quality improvement and performance improvement within pharmacy operations.
• Maintain accurate clinical documentation and ensure compliance with applicable policies and standards.
• Independently manage assigned workload while meeting established productivity expectations.

Typical Day

The pharmacist will primarily conduct medication reviews , analyze member medical information and pharmacy claims, and identify gaps in care and drug therapy problems. The role requires a high degree of independent clinical judgment, attention to detail, and critical thinking.

Performance Expectation: Approximately 90% daily productivity .

Required Qualifications

• Bachelor of Science in Pharmacy (B.S./B.S.Pharm) or Pharm.D.
• Current, unrestricted Pharmacist license in the candidate's state of residence .
• Minimum 2 years of Medication Therapy Management (MTM) experience .
• Strong clinical pharmacy knowledge and medication-review experience.
• Ability to independently evaluate clinical information and pharmacy claims.
• Strong critical-thinking and clinical decision-making skills.
• Ability to work independently in a remote environment.
• Strong written and verbal communication skills.
• Excellent attention to detail and organizational skills.
• Ability to meet productivity and quality expectations in a high-volume environment.

Preferred / Additional Experience

• Managed care pharmacy experience.
• Medicare and/or Medicaid pharmacy experience.
• Prior Authorization experience.
• Drug Utilization Review (DUR).
• Formulary management and clinical criteria development.

Important Disqualifier

Candidates without Medication Therapy Management (MTM) experience should not be submitted.

Candidates must be authorized to work in the United States without current or future employment-based visa sponsorship . Sponsorship is not available for this position.

Candidate Profile

The strongest candidate will be a licensed pharmacist with 2+ years of direct MTM experience , preferably within a managed care, Medicare, Medicaid, or pharmacy clinical-program environment. Candidates should be comfortable independently reviewing medical records and pharmacy claims, identifying drug therapy problems and care gaps, and documenting clinical interventions in a remote setting.
