Primary Care Physician-1

Position Overview Primary Care Physician (PCP) responsible for comprehensive, patient-centered care for a panel of up to 450 older adults. The role centers on accountability for outcomes, coaching patients for behavior change, and simplifying care processes while working independently and collaborating with hospitals, care teams, and Advanced Practice Practitioners. Key Responsibilities Accountability for outcomes: demonstrate accountability for patient outcomes, deliver strong clinical care, and maintain cost-effectiveness for each patient in the panel. Coaching for health: act as a health coach, helping patients set short- and long-term health goals, partner with patients to work toward those goals, and follow up frequently to improve health. Simplifying for action: simplify and prioritize appropriately so behavior change is actionable for patients and for leading care teams toward performance goals. Provide care for acute and chronic illnesses in older adults, including assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. Engage specialist consultation when needed, remain involved in detailed patient care, and collaborate with hospitalists when patients are hospitalized. Lead care team consisting of care promoter, care facilitator, and care coordinator; engage transitional care teams for patients who cannot visit the office. Participate in risk and quality management programs, clinical meetings, and other meetings to promote patient health and company goals. Adhere to departmental goals, regulatory compliance, quality care standards, and company policies. Cover for other providers as needed and assist in recruiting patients for the center and additional providers for the company. Collaborate with Advanced Practice Practitioners (APP) and remain accountable for the APP’s actions while employed with the company. Qualifications MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty. Current, active State medical license in the state(s) of employment. Board certification or board eligibility in specialty; maintain certification through MOC, CME, or retake exams. Current DEA number for schedule II‑V controlled substances. Basic Life Support certification within the first 90 days of employment. Proficiency with Microsoft Office Suite and related software. Fluent in English, verbal and written; additional language fluency may be required based on center location. Availability for local travel, national training, and occasional regional or national recruiting. Willingness to work evenings, weekends, and holidays as needed. Strong patient availability, including walk-in visits and prompt phone/messages response. Commitment to evidence-based medicine, leadership, quality improvement, influence, and self‑care practices. Compensation and Benefits Pay range: $221,141 – $315,915 (base salary, may include bonuses or commissions). Compensation depends on experience, education, location, and other factors.
