Care Manager (RN) - Outpatient

Scope of Position / Position Summary The Patient Care Resource Manager, PCRM, is a nurse case manager that facilitates patients through the health care system to include pre‑admission through post‑discharge coordination of care, resources and services utilizing the nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput and to enhance care quality and efficiency and ensure efficient resource utilization. The PCRM identifies and removes barriers to progression of the plan of care for each patient. Minimum Education Required Bachelor's Level Degree or equivalent combination of education and experience. Required Qualifications Bachelor’s degree, Master’s preferred. RN licensure required; five years recent clinical experience applicable to the population preferred; knowledge of clinical practice guidelines, computer databases, and payer protocols desired.
