Ability to work independently in home health setting


Description:
Provide high-quality nursing care to patients in hospice.
Assist patients and families with end-of-life decision making.
Manage pain and other symptoms to ensure comfort.
Coordinate with interdisciplinary teams to provide holistic care.
Educate patients and families regarding care plans and resources.
Document patient progress and changes in condition accurately.
Adhere to ethical and legal guidelines in all aspects of care.
Support family members and caregivers during the grieving process.
Maintain a safe and respectful environment for patients and families.
Demonstrate a commitment to ongoing professional development in hospice care.
Participate in regular team meetings and training sessions.
