Patient Care Coordinator RN

Kaiser Permanente seeks a part-time Patient Care Coordinator RN at Virginia Hospital Center to support patients across the care continuum. You’ll complete assessments, develop and update care plans, coordinate appointments and referrals, and ensure smooth transitions between inpatient, outpatient, and virtual care. Using modern IT tools and EMRs, you’ll document progress, analyze patient data, and collaborate with physicians, therapists, and social workers. You’ll educate patients and families, support chronic disease management, and contribute to quality and process-improvement initiatives in a highly collaborative culture.
