The Home Health RN performs comprehensive assessments, develops and implements individualized plans of care, provides skilled nursing interventions, and partners closely with physicians and interdisciplinary team members to promote safety, independence, and optimal outcomes. The Home Health Registered Nurse (RN) plays a critical role in delivering skilled nursing care and comprehensive case management services to patients in their homes. In this role, the RN functions as both a clinician and a case manager—conducting holistic assessments, developing and managing individualized plans of care, coordinating services across the interdisciplinary team, and monitoring patient progress throughout the episode of care. The Home Health RN serves as the primary point of clinical oversight, ensuring care is delivered safely, efficiently, and in alignment with physician orders, regulatory requirements, and patient goals, while supporting successful outcomes and transitions across the continuum of care.**Pay Range:** $34.00 - $49.18/hour**Employment Type:** Full Time, 72 hours every 2 weeks.**Schedule:** Day shift, Typically 8 hour days.
