Job Description
Title: Skilled Nursing - Rehabilitation

Schedule:
Shift: 08:00 AM - 04:30 PM
Days: Monday - Friday
Weekend rotation required.

Assignment Details:
Contract Length: 52 weeks
Guaranteed Hours: 0

Requirements:
- Must have a valid nursing license.
- Relevant certifications may be required.
- Previous experience in skilled nursing or rehabilitation preferred.

Roles & Responsibilities:
- Provide patient care and support to individuals in a rehabilitation setting.
- Assess patients' needs and develop appropriate care plans.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Monitor patients' progress and adjust care plans as necessary.
- Educate patients and families regarding care plans and rehabilitation processes.
- Maintain accurate documentation of patient care activities.

Compliance / Notes:
- Background checks and drug screens may be required.
- Orientation rules apply for new staff.
