Serenity Spring Senior Living at Northwood is a skilled nursing facility, providing clinically sophisticated sub-acute care and rehabilitation, as well as comprehensive nursing care. Coordinates medically related social services to nursing home residents overseeing the admissions and discharge process.
Duties include:
- Execute admitting individuals to facility working with local referral sources and community members
- Perform cognitive and psychosocial assessments and care planning
- Discharge planning to include making referrals to community resources
- Providing emotional support and education to residents and their family members
- Advocating for residents’ rights
- PHQ-9
Qualifications:
- Must possess a bachelor’s degree in a human service field.
- Experience working in a nursing home setting preferred.
