Physician Assistant

Overview Nest Health is a value-based care provider delivering family-centered in-home and virtual care, including primary adult and pediatric care, behavioral health and nutrition support, vaccinations and labs, same- and next-day acute visits, chronic condition and transitions of care management, social services and navigation, and more. Role The Physician Assistant (PA) is the primary care provider delivering whole-family primary, acute, chronic, and pediatric care in the home setting, collaborating closely with a clinical team of physicians, behavioral health counselors, registered dietitians, and a medical assistant upskilled to support social services navigation. What will you do? Manage a panel of families as the primary care provider to improve whole-family health through longitudinal primary and preventive care, care management, and care coordination in the home and via telehealth (video, phone, SMS). Practice with an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists, dietitians, and medical assistants. Utilize Nest Health’s electronic health record and care management tools to collect data, document member interactions in the field, organize information, track tasks, and communicate with your team, members, and community resources. Attend daily team huddles and weekly case conferences to advise on physical health needs and establish health goals as part of the Nest Action Plan (NAP). Propose treatments for chronic and infectious diseases, prescribe and reconcile medications, and educate patients about their conditions and medications. Order, administer, obtain, and record diagnostic tests. Provide information and assistance through advocacy, navigation support, and education to Nest families on health and social care services. Participate in outreach to Nest families, including explaining program details to potential families. What do you bring to Nest? Completion of a Physician Assistant or Nurse Practitioner accredited program and active, unrestricted license to practice in Arizona. Experience (at least 2 years) providing clinical services to pediatric (including newborns) and adult patients with both chronic medical and behavioral health conditions, and interest in serving underserved, vulnerable populations. Proficiency or willingness to train in clinical nursing skills such as venipuncture, patient assessments, and care plan reinforcement for common adult and pediatric chronic conditions. Proven ability to make independent clinical decisions and problem-solve. Available to work full-time, five days per week, and participate in a rotating call schedule that includes occasional evening and weekend days. Maintains current CPR certification (BLS). Experience leading an interdisciplinary care team preferred. Experience in a startup environment is a plus but not required. Access to reliable transportation and willingness to comply with the Nest Motor Vehicle Safety Policy. Note: This position involves educating and encouraging vaccination against COVID-19. Travel & Physical Requirements This is a mobile role providing service in family homes. Must have a driver’s license, a car (mileage reimbursed), auto insurance, and the ability to drive to Nest family homes with portable supplies.
