Key Responsibilities: Develop, implement, and monitor social service programs that meet all regulatory requirements Support discharge planning, resident assessments, and individualized social care plans Collaborate with residents, families, healthcare providers, and community organizations Coordinate department functions and manage social services staff Ensure compliance, resident rights, confidentiality, and quality standards Participate in quality improvement efforts and handle social services emergencies Qualifications: Bachelor’s degree in Social Work or a related field (required) Experience in long-term care or healthcare setting preferred Strong communication, problem-solving, and leadership skills Compassionate approach and ability to advocate for residents If you’re looking for a meaningful career in a supportive and welcoming environment. #J-18808-Ljbffr
