As a member of our team, you'll have the opportunity to make a positive impact on the lives of patients while enjoying competitive pay, comprehensive benefits, and the support of a dedicated clinical team.Required Qualifications:One year of recent experience as an LPN, including a minimum of six months in a Home Health setting as an LPNValid LPN license in compliance with state regulationsCurrent BLS (AHA/ARC) CertificationPreferred Qualifications:Other certifications and licenses may be required for this positionSummary:A Home Health Licensed Practical Nurse (LPN) delivers direct nursing care to patients in their homes, ensuring comfort, stability, and effective management of their medical conditions. Home Health LPNs follow care plans, administer medications, assist with treatments, and monitor patient progress under the supervision of an RN or physician.Essential Work Functions:Deliver direct patient care in accordance with individualized care plans, physician orders, and nursing protocolsAdminister medications and treatments, including injections, wound care, and monitoring for side effectsAssist in educating patients and caregivers on proper medication use, treatment plans, and home care techniquesObserve and report changes in patient conditions to the supervising RN or physician for timely interventionCollaborate with RNs, therapists, and other healthcare providers to ensure seamless, high-quality patient careMaintain accurate and timely documentation of all care provided, patient responses, and any concernsEnsure compliance with infection control, safety, and home care best practices to protect patients and caregiversSupport families and caregivers, offering guidance and emotional support throughout the care processMaintain communication with case managers and interdisciplinary teams to provide coordinated patient careComplete charting and documentation requirements while meeting visit and productivity goalsAdhere to infection control, safety, and healthcare regulationsPerform other duties as assigned within the scope of practiceRequired Essential Skills:Critical thinking, service excellence, and good interpersonal communication skillsThe ability to read, write, and communicate in the English languageAbility to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skillsPhysical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bendInterpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalismTechnical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipmentMental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel.
