Safe Life Home Health Care, LLC is seeking an office-based Clinical Case Manager to support referral intake, case triage, care coordination, and communication with patients, families, referral sources, and the clinical team. This role is ideal for an RN, LPN, or LVN who is organized, responsive, and comfortable managing a high volume of incoming cases in a fast paced home health setting.
