TVC/IFC CoordinatorTVC/IFC Coordinator serves as the primary point of contact for Veterans, their families and caregivers, and interdisciplinary healthcare teams. Responsibilities and duties are but not limited to the following:Demonstrate proficiency in practice based on conscious and deliberate planning.Self-directed in goal setting for managing complex client situations.Assumes responsibility for the coordination of care for traveling Veterans either vested in another Veterans Affairs (VA) facility and traveling to the local catchment area.Focus on patient education, self-management, and customer satisfaction throughout the continuum of care.Influences care outcomes by collaborating with members of the interdisciplinary team.Ensures effective communication and continuity of care throughout all phases of patient movement, referral, and care transitions both within and between VHA sites of care.Oversight of interfacility consults, including triage, processing, and coordination of all requisite pre-consult diagnostic testing, labs, and imaging, as well as followup diagnostic care as necessary.The incumbent utilizes clinical practice guidelines, care pathways, and Milliman Care Guidelines (MCG) to ensure care is provided in the least restrictive and most cost-effective manner while monitoring chronic disease and health promotion indices through clinical guidelines and performance measures.The TVC/IFC Coordinator is further responsible for performing quality and performance measures analysis and reporting trends related to consults, referrals, and systems issues utilizing the nursing process.Functions as an emerging leader in providing direction to staff to identify, implement and evaluate patient care needs.Collaborates with the interdisciplinary team to ensure the plan of care is achieved.VA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: Monday - Friday; 8:00 a.m. - 4:30 p.m.Telework: Ad-hoc only and based on agency policy, not regular or reoccurringVirtual: This is not a virtual position.Relocation/Recruitment Incentives: Not authorizedPermanent Change of Station (PCS): Not authorizedEDRP: Not Approved
