$6,000 sign on bonus Under the general direction of the Director of Admissions and Palliative Care Services, the Admission Registered Nurse (RN) is responsible for determining the eligibility and preparation of patient/family needs and the initial written plan of care. The role works closely with the Intake Coordinator, Clinical Service Department, and the Chief Operating Officer to ensure timely processing of all referrals. The RN serves as a resource for questions and issues from the health care community and SFR staff. The position is full time and will include working weekends. Territory is Central/South County 12 pm - 10:30 pm - 4 shifts per week Reports to: Director of Admissions and Palliative Care Services Directly supervises: None Education and Training Associate degree in nursing. Bachelor’s or Certified Hospice and Palliative Care Nurse preferred. Certifications and Licenses Valid Florida Registered Nurse License. Valid Florida Driver’s License with reliable transportation. Experience, Skills, and Abilities Three years hospice experience (preferred). Previous sales experience preferred. Ability to work flexible hours. Understanding of the dynamics of human behavior and ability to communicate with patients and families in a crisis. Effective communication with referral sources and colleagues. Excellent oral and written communication, clinical and administrative skills. Ability to prioritize varied projects. Dependable, flexible and tactful. Physical Requirements Ability to lift, push, pull 25 pounds without difficulty. Good visual and audio acuity. Ability to work extended hours as required. Ability to work under varying levels of stress. Primary Responsibilities Assist the Intake Coordinator with the coordination of referrals. Meet the needs of the newly referred patient/family, completing the admissions process within 24 hours or sooner. Give a comprehensive explanation of hospice services, philosophies, and benefits addressing all components of care. Obtain signatures on all necessary consents ensuring that patient/families are informed of their rights and responsibilities. Perform a physical, environmental, psychosocial and spiritual assessment. Provide primary clinical care and teaching, as needed, to stabilize the patient and facilitate the admissions process. Document assessments and establish the initial plan of care and plan of treatment after consultation with another team member. Be available and flexible with scheduling to meet the admission needs of the patient/family. Be an active, enthusiastic member of the admissions team providing support for colleagues and creative ideas for improvement of the admissions process. Maintain strict confidentiality and respect for patient/family privacy and possessions. Provide emotional support to patient/family and other caregivers. Increase knowledge of palliative care through active participation in educational opportunities. Assist patient/family with understanding requirements during implementation of a disaster plan. Participate in orientation and education of SFR personnel. Participate in performance improvement and quality control activities. #J-18808-Ljbffr
