Case Manager - BH

Location & Status Location: Hernando County Position: Full‑Time, Hourly Shift: Days (varying hours, occasional weekend work, on call) Job Description Provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, or involvement in the child welfare system. Demonstrate competency in engagement and assessment, service planning and delivery, coordination, linkage and monitoring, documentation, and professional responsibility. Coordinate service delivery and provide community partners with up‑to‑date referral resources. Maintain and manage assigned caseload based on individuals' needs and preferences. Understand the service delivery system and comply with regulations and district policy. Provide services at appropriate time and location; travel required. May conduct urine drug screenings and transport clients as required. May participate in a 24‑hour on‑call rotation and perform other duties as assigned. Required Qualifications Bachelor's degree in Social Work, Counseling, Psychology, Criminal Justice, Health Education, Special Education or a related human service field. 1 year full‑time experience working with children with behavioral health concerns (for child‑specific roles) or adults with behavioral health concerns (for adult‑specific roles). Basic Life Support (BLS) certification. Valid driver's license. For targeted case managers: CBHCM certification through the Florida Certification Board within one year of employment. Preferred Qualifications Experience billing Medicaid for targeted case managers. Experience with targeted populations as indicated. Benefits Health, dental, and vision coverage. Paid time off.
