**LPN - Evening Shift**

* Provide skilled nursing care to residents under the supervision of the Registered Nurse (RN)
* Administer medications, treatments, and therapies as prescribed by physicians
* Assist with daily living activities such as bathing, dressing, and grooming
* Monitor and report changes in residents' conditions to the RN
* Collaborate with the interdisciplinary team to develop and implement individualized care plans
* Maintain accurate and detailed records of patient care
