Home Health Aide (HHA)Facility: Spring Creek Rehabilitation & Nursing Care Center Location: Brooklyn, NY Position: Home Health Aide (HHA) Schedule: PRN Pay: $19–$20 per hourJoin Our Team!Spring Creek Rehabilitation & Nursing Care Center is seeking a dependable and compassionate Home Health Aide (HHA) to assist residents with safe transportation to clinic and dialysis appointments and provide support throughout their visits.ResponsibilitiesEscort residents safely to and from clinic and dialysis appointments.Arrive approximately 1 hour before scheduled appointments.Assist residents with preparing for appointments and provide approved snacks for out-on-pass visits.Monitor residents for safety and help prevent falls or incidents.Report accidents, incidents, or concerns promptly to the Clinical Coordinator or ADNS.Ensure required medical records and paperwork accompany residents.Verify DNR/DNI status and required documentation.Communicate clinic concerns and advocate for residents.Coordinate assignments and appointments with the Clinic Coordinator.Follow all facility policies, safety procedures, and abuse-prevention regulations.May be required to attend multiple appointments in one day.RequirementsCurrent NYS HHA certification required.CNA certification preferred.High school diploma or equivalent preferred.Previous nursing home or healthcare experience preferred.Strong communication and interpersonal skills.Professional appearance and dependable work ethic.Ability to prioritize resident safety and follow instructions.Apply Today!Join our team and make a meaningful difference in the lives of our residents.
